Printed and handwritten inscription: "Certificate of Baptism. / This Certifies, / That Della La Grange Van Name, Infant, daughter / of Paul M. and Susie A. Van Name, born in / Tottenville S.I. on the 22nd day of November, 1879, / having been presented for Holy Baptism, and it having / been solemnly promised that she shall be taught the nature and / end of this Holy Sacrament, and all other things which a Christian / ought to know and believe, in order to a virtuous and holy life, was / this day baptized by me, in the name of the FATHER, and of the / SON, and of the HOLY GHOST, according to the Discipline and / Usages of the METHODIST EPISCOPAL CHURCH. / Lewis A. [Dunn?] / Presiding Elder / Tottenville S.I. June 12, 1881." Handwritten inscription in pencil on reverse: "Paul R. Van Name Esq / Tottenville SI".
